How to Apply

In order to apply for a U.S. passport you must provide the following:

All applicants must be present when applying (including minor children)

Completed Application for a U.S. Passport (DS-11)  - BLACK INK ONLY

Submit original OR certified copies of U.S. citizenship:

1. Certified birth certificate*
2. Previous Passport
3. Certificate of Naturalization

 * California Abstracts of Birth, Hospital Birth Certificates & Birth Registrations are unacceptable proof of citizenship


One (1)  Passport photo (see list of available Passport photo providers) 


Valid photo identification:

1. Driver’s License
2. Military Identification
3. Passport
4. Government photo identification

Fees:     NO CASH

             Check or Money Order for Passport is made payable to:  U.S. Department of State
             Check or Money Order for City fees are made payable to:     City of Visalia
